The forms of application for permit provided for in Section 15.08.030 shall require the following information, and such additional information as is found by staff to be necessary to carry out this chapter:

1. Name and address and phone number of the applicant and if by a corporation of the officers thereof;

2. A description of the park or plaza location at which it is desired to conduct such business;

3. A copy of a current city business and occupation license, or proof of application therefore, together with proof of payment of B & O fee or, in case a B & O license is not required, evidence as to why it is not required;

4. Type of any items to be sold;

5. Description of how business will be conducted, and a drawing of the vehicle or stand from which goods will be sold, to show its size, color, signage, etc., and a description of means by which goods will be transported to and from the site;

6. A statement to be signed by the applicant agreeing to hold harmless the city and its officers and employees from any claims for injuries or damages alleged by any person to have been caused by such activity;

7. If goods are to be sold from any stand or cart, proof of insurance in the amount of three hundred thousand dollars to cover claims for injury or damages suffered or alleged to have been suffered by any person as a result of such activity, which insurance policy shall name the city as an additional insured and shall provide it cannot be canceled except after ten days' written notice to the department of Vancouver/Clark Parks and Recreation;

8. Proof of application for all permits required by other public agencies such as the Health District. (Ord. M-2480 §3, 1984)
