Except as provided elsewhere in this chapter, the application for a special event permit shall include the following:

A. The name, address, telephone number, cell phone number, facsimile number and e-mail address of the applicant.

B. A certification that the applicant will be financially responsible for any City fees or costs that may be imposed for the special event.

C. The name, address, telephone number, cell phone number, facsimile number and e-mail address of the event organizer, if any, and the chief officer of the event organizer, if any.

D. A list of emergency contacts that will be in effect during the event, and the event web address, if any.

E. If the special event is designed to be held by, on behalf of, or for any organization other than the applicant, the applicant for the special event permit shall file a written communication from such organization:

1. Authorizing the applicant to apply for the special event permit on its behalf;

2. Certifying that the applicant will be financially responsible for any costs or fees that may be imposed for the special event; and

3. A copy of the tax exemption letter issued by the United States Internal Revenue Service for any applicant claiming to be a tax–exempt nonprofit organization.

F. The applicant is responsible for submitting all information required for the application for a special event. Unless otherwise exempt under this chapter, all permit applications should include the following:

1. A statement of the purpose of the special event.

2. A statement of fees to be charged for the special event.

3. A detailed description of the proposed location for the special event, including documentation of the right to use the property.

4. Dates and times when the special event is to be conducted.

5. The approximate times when assembly for, and disbanding of, the special event is to take place.

6. The proposed plan for garbage and recycling collection.

7. The proposed locations of the assembly or production area.

8. The specific proposed site or route, including a map and written narrative of the route.

9. The proposed site of any reviewing stands.

10. The proposed site for any disbanding area.

11. Proposed alternate routes, sites or times, where applicable.

12. Information documenting compliance, if needed, with the Americans with Disabilities Act (ADA) requirements or accessibility, parking and restrooms.

13. The approximate number of persons, animals or vehicles that will constitute the special event.

14. The kinds of animals anticipated to be part of the special event.

15. A description of the types of vehicles to be used in the special event.

16. The number of bands or other musical units and the nature of any equipment to be used to produce sounds or noise.

17. The number, type and location of portable sanitation facilities.

18. Other equipment or services necessary to conduct the special event with due regard for participant and public health and safety.

19. The number of persons proposed or required to monitor or facilitate the special event and provide spectator or participant control and direction for special events using city streets, sidewalks, or facilities.

20. The anticipated quantity and types of any alcohol that will be served at the event, for which a separate Washington State Liquor Control Board permit is required.

21. Provisions for first aid or emergency medical services, or both, based on special event risk factors.

22. A traffic control plan, if the event will occur on a public street or sidewalk, or will otherwise impact vehicle or pedestrian traffic.

23. Liability insurance documentation, including the certificate(s) of insurance.

24. A Hold Harmless and Indemnity Agreement in such form as may be issued by, or acceptable to, the city’s risk manager.

25. Any special or unusual requirements that may be imposed or created by virtue of the proposed special event activity.

26. Any other information required by the responsible official.

G. The responsible official will notify the applicant within fifteen (15) business days of receiving a special event permit application of any information that the applicant has failed to provide or is incomplete. (Ord. M-3956 §1, 2010)
