5.53.060 License application--Report by city department.
A. Any person seeking to acquire or to renew a panorama premises or panorama device license shall file an application with the city clerk on a form approved by the clerk for that purpose.
1. The applicant must prepare and sign the application. If the applicant is a partnership, a partner must sign and the names of the partners be given with their current addresses. If the applicant is a corporation, an authorized officer thereof must sign and the names and current addresses of all officers and directors of the corporation shall be given, together with the location of the principal office or place of business of such corporation. All corporations must designate an agent to receive service for said corporation.
2. The clerk shall refer such application to the chief of police, and the chief shall cause a full investigation to be made as to whether applicant and the premises comply with provisions of this chapter. The clerk also shall refer the application to Community and Economic Development, and to the Vancouver fire department for their report as to fire, zoning and other codes compliance.
B. The following information shall accompany each such application for a license:
1. Panorama Premises License.
a. The name, address and telephone number of person applying for the license;
b. The name and address and telephone number of each person holding an ownership, leasehold or interest in the panorama premises;
c. The name, address and telephone number of the manager or other person responsible for the operation of the premises;
d. The address of the premises;
e. The number of panorama devices intended to be located on the premises;
f. A sketch or drawing sufficient to show the layout of the premises, including all information necessary to determine whether the premises will comply with the provisions of this chapter and specifically of Section 5.53.090.
2. Panorama Device License.
a. The name, address and telephone number of each person applying for the license;
b. The name, address and telephone number of each person holding an ownership, leasehold or other interest in the panorama device;
c. The address at which the panorama device is to be located;
d. A description, including make, model and serial number of the device. (Ord. M-3068 §7, 1993)
