In addition to, or as an alternative to any other penalty, there shall be assessed a civil penalty of one hundred ($100) dollars for failure or refusal to obtain or renew an alarm registration, or to pay the alarm registration fee therefore as required by this chapter. The chief of police and his or her designate is authorized to issue a notice of civil violation assessing such civil penalty against any person or persons reasonably believed to have failed or refused to obtain or renew an alarm registration or paid the fee therefore as required by this ordinance. The notice of civil violation shall include the following:

1. The name and address of the person or persons to whom the notice is directed;

2. The street address of the premises at which the alarm is being maintained without the required alarm registration;

3. A concise statement describing the nature of the violation;

4. A statement that the determination of violation must be appealed to a hearings examiner within ten days of receipt of the notice or the determination shall be deemed final and appeal rights deemed waived. An appeal request shall include a twenty five ($25) dollar fee which may be refunded upon the hearing examiner's determination that no violation occurred.
