A. All wastewater discharge permit applications and reports must be signed by an authorized representative of the User and contain the certification statement in Section 14.10.430(A).

B. All survey forms must be signed by an authorized representative of the User and may contain the certification statement in Section 14.10.430(A).

C. Users shall submit a new authorization if the designation of an authorized representative is no longer accurate. This includes when a different individual or position has responsibility for the overall operation of the facility, or overall responsibility for environmental matters for the company. The User must submit the new authorization to the City prior to or with any reports to be signed by the new authorized representative. (Ord. M-3970 §2, 2010)
